Creating an AWARE Community
Pet Health Hub is an initiative of AWARE Foundation NPC, created to make responsible pet ownership and essential pet healthcare more accessible while creating a positive impact for animal welfare.
Through AWARE Community Days, pet owners can access FREE community pet health services provided by EarthPet Vet, while the FREE Pet Health Passport web app gives owners a convenient digital place to store and manage important information about their pets.
The initiative also supports AWARE Foundation's wider animal welfare activities, extending its impact beyond the pets attending Community Days to animals and rescue organisations in need.
We donate towards responsible, environmentally conscious after-life care for pets, helping keep deceased animals out of landfills while promoting sustainable alternatives that protect the environment and honour animals with care, respect and dignity beyond life.

Free pet healthcare for the community
Our AWARE Community Days are designed to make important preventative pet healthcare more accessible while promoting responsible pet ownership and dignified animal care. We prepare to provide service to 125 pets at an AWARE Community day.
At our first Community Day, pre-registered cats and dogs can receive:

You can donate or purchase merchandise at the AWARE gazebo afterwards, with your support contributing to sustainable animal welfare.
IMPORTANT
Please note that no health checks or veterinary consultations are conducted at AWARE Community Days. Veterinary services are strictly limited to microchipping and vaccinations.

Services are available by pre-booking after registering on the Pet Health Passport Web App for FREE, helping us manage attendance and ensure the event is appropriately staffed and equipped for the pets attending.
We donate microchip scanners
For every 125 microchips placed at our AWARE Community days, we donate 7 microchip scanners to participating veterinary practices, animal welfare and rescue organisations and government institutions.
This helps put reliable scanning equipment in the hands of organisations that can use it to identify more lost and found pets and help reunite them with their owners.
We carefully select recipient organisations to ensure every donated scanner is placed where it will be used responsibly and effectively.

Important pet health records are still often kept across vaccination cards, certificates and separate paper documents that can easily be misplaced, damaged or simply become difficult to find when they're needed.
The Pet Health Passport is a web application designed to give pet owners one convenient digital place to maintain important information about their animals from any web browser.
Your Pet Health Passport can bring together information including:

Keep your pet's important identifying information together.
OWNER DETAILS
Maintain the owner information associated with your pet.
MICROCHIP INFORMATION
Keep your pet's microchip details and microchipping history accessible.
VACCINATION RECORDS
Maintain a convenient record of your pet's vaccinations.

Record whether your pet has been sterilised.
IMPORTANT HEALTH INFORMATION
Keep relevant pet health information together instead of scattered across paperwork.
CONNECT A MICROCHIP NUMBER TO YOUR PET PASSPORT
A tiny chip can make a big difference.
A microchip provides your pet with a permanent form of identification and increases the chance of a lost pet being identified and reunited with you. Microchips are small. We use small 2.12mm thick x 12mm long microchips for cats and dogs, and smaller 1.25mmx 7mm microchips for smaller pets.
With the free version of the Pet Health Passport, your pet's microchip number is linked to their unique Pet Health Passport number. Any microchip that scans can be linked to the the web app. If your pet is found and identified through their microchip, this connection can help us notify you.
You remain in control of your pet's information. Choose what information you want to make available and adjust your privacy settings according to your preferences. You can also choose to make your pet's vaccination records available to your veterinarian when needed.
A microchip can also provide answers in more difficult circumstances. If a deceased pet is found, identification through their registered microchip can help provide their family with certainty and the opportunity for a dignified farewell.
